In order to complete your GEL Registration form, you will need to upload a signed GEL Day Student Release Form. If you are under 18, a parent or guardian signature is required.
GEL Day Student Release Form
Instructions for Uploading your GEL Release Form:
- Download the document (linked above)
- Print the document
- Complete the form with the student's information
- Scan completed document to computer or take a photo of both pages of the document
- When filling out your GEL Registration, when prompted, upload the scan or photo
When reviewing your completed GEL Student Release Form, here is what you should look for:
- Students may sign the release form on their own if they will be 18 years old by GEL Day
- Date of birth must be on or before 4/15/2005
- Clear and legible release form
- Both the front and back pages are included
- The student's name and date of birth at the top
- It is signed, dated and initialed
- Initials are required at the bottom right of each page of the waiver
- Preferred document type: PDF or JPEG
This form will need to be uploaded to your GEL Registration. The uploader will only accept one document. If you cannot upload both sides, or would prefer to email, please submit your registration form and email the release form to gel@gonzaga.edu. If you have any questions, please contact us.
