Leave of Absence Policies
In accordance with the Federal Family and Medical Leave Act (FMLA), the University's policy is to grant up to 12 weeks of unpaid leave during a 12-month period. Employees applying for Family and Medical Leave must complete a Leave of Absence Request Form, accompanied by the Certification of Health Care Provider form provided below:
