Liability Release form

Each person participating in an intramural activity needs to fill out a liability release form with your insurance information COMPLETED in case during emergencies you are unable to provide your personal information.  All liability release forms are due with the Team Roster sheets.

Note: Multiple liability release forms need to be completed and turned in with each team an individual plays on.
