• If you have never had the Hep B immunization series, you should start getting the three required doses now. The 1st dose is followed by the 2nd dose one month later. The 3rd dose is given at least two months after 2nd dose and at least 4 months after 1st dose. Schedule a titer to be done 6-8 weeks after the 3rd dose. You need to complete and show proof of at least two doses in order to be able to participate in your practicum courses. Then you must follow through to complete the series and the titer. If your titer is positive, you have met the requirements as long as you have provided a copy of the lab report or healthcare provider documentation of the result.
• If you completed the immunization series, obtain a titer now to determine immunity status. If you do not have a record of your immunizations, you need to have a titer drawn to document your immunity status and submit a copy of the lab test result. If your titer is positive, you have met the requirements as long as you have provided a copy of the lab report or healthcare provider documentation of the result.
• If your titer is negative (non-immune), you need to show proof of receiving the series of three Hep B immunizations. You also need to repeat the series of vaccinations and another titer to determine immunity. If your titer is positive, you have met the requirements as long as you have provided a copy of the lab report or healthcare provider documentation of the result.
• If you are not able to locate your records documenting proof that you received the Hep B immunization series and you have a negative titer, you must repeat the series of vaccinations and another titer to determine immunity. If your titer is still negative, you must repeat the series again and the titer. If your titer is still negative, you are advised to consult with your primary healthcare provider regarding appropriate precautions.
• If you decline the vaccination, you must provide a signed waiver.

You must complete 1 or 2 below (not both):

1. A current two-step TB test within the last year or a previous 2-step TB test with subsequent annual tests given before the annual deadline.

• 1st Step: Administer and read 48-72 hours later
• 2nd Step: 1-3 weeks after the first test a second test is done. Administer and read 48-72 hours later. Provide copies of both tests.

If you interrupt this process and spread the tests out too far, you will have to re-do the entire process so pay attention to timelines.

2. Quantiferon Gold Blood test (lab report required) in the past 12 months.

If the results are positive, also submit a clear chest x- ray (with report) or healthcare provider documentation of the result AND documentation of a negative symptom screen AND documentation of the positive test result.

You must have proof of TWO doses of vaccine a minimum of 28 days apart.

If you have not had two doses, start getting them now. If you do not have a record of your immunizations, you need to repeat the vaccinations or have a titer drawn to document your immunity status. Provide a copy of the immunizations or titer report or healthcare provider documentation of titer result. If your titer does not show immunity to all three components and you have documentation of two doses of vaccine, you are considered to have “presumptive evidence of immunity and do not need to repeat the vaccine.

You must complete 1 or 2 below (not both).

1. You must have proof of TWO doses given 4-8 weeks apart. If you have not had two doses, start getting them now and provide documentation.

2. Have a blood draw (titer) done to document immunity status if you have had the disease. If the titer is negative and you have documentation of two doses of vaccine, no additional vaccines are required.

Provide a lab report or healthcare provider documentation of the titer result.

You must have had at least 1 Tdap given after the age of 12 or within the last 10 years (DTap, DTP, DT, TD are not sufficient immunizations).

If you have not had a Tdap immunization, you must get one and provide documentation. A Td is then required every 10 years.

All clinical faculty are required to obtain an annual flu vaccine.
