IRB Forms
In order to submit a proposal to the IRB you must fill out (1) the appropriate cover sheet and (2) the full application. Submit these documents to Dr. Deborah Booth, boothd@gonzaga.edu, AD Box 25. (Electronic submissions are required.) The IRB will meet during Academic Year 2012-2013 on September 17, October 22, November 19, December 10, January 28, February 18, March 18, and April 15. Meetings will be held from 8:00-9:00am in Rosauer Room 260. Applications received two weeks in advance will be reviewed.
