Leave of Absence Policies

/In accordance with the Federal Family and Medical Leave Act (FMLA), the University's policy is to grant up to 12 weeks of unpaid leave during a 12-month period. Employees applying for Family and Medical Leave must complete a Leave of Absence Request Form, accompanied by the Certification of Health Care Provider form provided below:


The University grants paid and unpaid leaves of absences depending on length of service, department work load, state and federal laws, and medical necessity. Please see below for forms and additional leave policies.
