This is the application to apply to graduate for undergraduate students. Your advisor's signature will be required prior to submitting the form to the Academic Credentials Services Office.
Your student account will be assessed a $50 degree application fee. Students who miss the application deadline will be assessed an additional $25 fee. Payment is due in the billing period for which such fees are incurred. Failure to pay this student account balance in timely fashion may result in additional late payment charges. The University reserves the right to withhold student information, including transcripts of record and diplomas, until said student's account has been paid in full.
