Assumption of Risk and Liability Release Form (this must be turned in for each player being added on the Replacement Player Form)
Replacement Player Form Requirements:
This form must be turned in with the Liability Release Form(s) for the "Replacement Player(s)" listed.
This form and the Liability Release Form(s) must be turned in prior to game day during office hours or no later than 12pm on game day at the Intramural Office located on the lower level of the Rudolf Fitness Center. No replacement players may play if the forms are not turned in on time to the Intramural Office.

Forms brought to the field will not be accepted.
If the forms are not completely filled out they will be considered not turned in.
All eligible players for tournament play must play in a minimum of one regular season game or be listed on the original roster to participate in tournament games.
Injury replacements will be taken on an individual basis.
Note: Any player removed from the Team Roster at any time cannot be placed back on that same Team Roster at any time.
Note: Completed forms will only be accepted at the Intramural Office prior to the deadline.
