Liability Release Form

Each person participating in an intramural sport/activity needs to sign the Assumption of Risk and Liability Release portion of the Registration Form. By printing and signing your name on the Registration Form you are agreeing to the Assumption of Risk and Liability Release and Code of Ethics portion of the Registration Form.

Anyone being added to a team using the Replacement Player Form is required to fill out the above Liability Release Form with your insurance information
COMPLETED in case during emergencies you are unable to provide your personal information.  All liability release forms are due with the Team Roster sheets.
