Roster sheet |
Completely fill out each team roster for each sport; there may be multiple teams from one floor. Make sure individuals are only playing on one all gender team. Men and Women floors combined for coed teams must stay together during the entire Freshman Games. |

Liability Release form |
Each individual listed on the Roster sheet must have a completed Liability Release form. To be on the safe side have all floor residents complete and sign a form. |
| The registration deadline, sport fee and location are located above. |
- Please place the documents in the following order:
- -Classification sheet
- -Roster sheet(s)
- -Liability Release forms in order as they appear on the Roster sheet.

The Intramural Department will send an email to each floor's RA with the total cost of Freshman Games along with a journal entry account number. The sport fee is $10.00 per team. |

Team Information |
- Captains serve as the contact between GU Intramurals and their team. It is important that captains know and follow their responsibilities accordingly.
- -Captains are responsible for team, individuals and spectators conduct.
- -See that all players and the team meet eligibility requirements (see Eligibility and Team Rosters).
- -New information arises often. Please continually recheck for updated information.
- -Ensure all team members know and adhere to the rules.
- -Inform team members of scheduled games and jersey colors.
- -Complete all needed paperwork for Replacement Player forms during the season.
